Dear Sir/Madam,

My PF number is MH / 93811/ 015. I had given my request for withdrawal of PF & have received acknowledgement dated 3rd June 2009. I have still not recived my PF the last when i visited PF office the never entrtained and we had a fight and later the next day i wrote this letter to PF department dated May 2010, still havent received my PF thats the reason i thought of contacting you.

My PF number is MH / 93811/ 015. I had given my request for withdrawal of PF & have received acknowledgement dated 3rd June 2009. When I last enquired with the PF office, Kandivali, I was not been entertained & was said that my account no is wrong, when I argued about account no that it is been given by some of your official she changed her statement & asked me to submit Form 9 which is supposed to be submitted by companies still I managed to bring a photo copy of Form 9 which was duly submitted by the company & company provided me acknowledged photocopy of the same(Form 9), yet the lady handling my PF account failed to attend my query neither she took my case seriously nor she gave me any kind of status report & behaved very shrudely. This case was already been delayed as she was always on leave whenever we contacted. Please let me know the actual status.
I want a clear answer from your department within 7 days & this letter should be treated as my final reminder & warning to the department(Especially Mrs. Kadam). I am staying in hospital at borivali and paying rent of 1600 + Food expenses 500 + opportunity cost 2000 daily + Mental harassment. Your department will be responsible for all the above expenses.

I will file case on 8th day to Consumer forum on Mrs. Kadam as well as on your department for non performing as well as having of such irresponsible staff working with your department.
