Sub: Non Receipt of Provident Fund

Claim form Submitted for me on 02.02.2009

I was working with M/s Futuristic Offshore Service & Chemicals Ltd. for the period
08.03.1989 to 15.04.2006. On resigning from there, applied for withdrawal of my
provident fund money immediately. Now after passing of nearly three and half years,
I have yet to receive my money. The number allotted to the company is Code No.
MH-40222/05.

I visited the Provident Fund office at Kandivli several times, but they are not
giving the proper answer i know the Employeer is fail to deposit the contribution.
But the P F department not taken any action against the Employeer.

After 28.08.09 i send a letter for RTI Kandivli office through Registerd post AD
they are received the same but till date they are not given any positive reply. Now
what I can do for the same.

Please advice me what i can do or what you can help in my case.

You are requested to look into the matter and arrange for clearance of my dues from
department at the earliest.

"If your bank account is in Pune, then your PF amount will be credited to your bank account after 4 months from date of submission & if your bank account is out of Pune, then your PF amount will be credited to your bank account after 5 months from date of submission."
