Dear Sir / Madam,


I, Chikkakambe Gowda working in Larsen & Toubro Ltd & I have been moved to USA on 6th July 2010 for onsite assignment.
When I was in India I have filled the passport application & signed the Annexure H & due to urgency I have been moved to USA. Hence I was not able to apply the passport for my son.
My wife Hemavathi R has applied passport for my son Vikas on 12th July 2010 at Main post office in Mysore & the passport file number is W18392110 & they have accepted the Annexure H along with the new passport application.
The police verification has been completed on 15th Sept 2010.
Me and my wife we were keep on trying to call Bangalore passport office, since passport status was not showing in your website.
We were not able to contact Bangalore passport office, due to all lines were busy.
Finally my wife got the status through tele enquiry (1800-258-1800) that our son passport is on Hold.
My wife visited Bangalore Passport office already 6 times for enquiry, first 3 visits they have not given any information. On 4th visit she met an officer by name Ramakrishna (APO) who is handling this file & he has asked her to bring a affidavit from consulate general of India, Chicago USA & he has informed that after getting this affidavit passport will be issued, since passport is in final stage. On 5th visit officer has seen the affidavit (attached for your reference) & forwarded to upload section. After uploading, the officer asked my wife to pay penalty due to Annexure H submitted & which is not required. My wife contacted penalty counter & they have asked her to come next day, since computer got hanged.
On 6th Visit person in penalty counter (PSC counter) verified the file & informed to my wife that they cannot receive the penalty due to no base to collect the penalty.
After that my wife contacted the officer he thought for a while & informed my wife that I have to get another affidavit from consulate general of India, Chicago USA to prove my signature.
My wife has travelled all the way from Mysore to Bangalore for all the 6 visits to Bangalore passport office (Koramangala).
Being a family person father is staying in one place, wife and son are staying in another place for close to 7 months due to long delay in passport is really painful.
I have already spent Rs 50000 as traveling expense to visit consulate general of India, Chicago, since the place where I am staying is away from Chicago.
The officer would have informed to get both the affidavits at one time.
It is painful to spend another Rs 50000.
We are totally confused.

I have attached Affidavit, Passport of mine & my wife, VISA & I-94 for your reference.

I kindly kindly request the passport officer to grant the passport for my son Vikas (DOB: 10/10/ 2006) 4 years of age, as early as possible.
