I Shiv Kumar Maurya S/o Shri Bali Ram Maurya R/o Vill-Bharsawan, Post-Damarua, Distt-Jaunpur (U.P.), applied for Pass Port through Agent near about 7 to 8 years ago, after some days having gone and LIU inspection had been done and then also Police Inspection through Police Station-Sikarara had been done but I have not received the Pass Port, hence once again I inquired after some months for passport and then one person reached at my home but there after no Pass Port had been issued to me. After some times I lost my Pass Port Application Acknowledgement Slip. When I could not received my Pass Port having gone some years then I applied for Pass Port once again in the month of May-2010 through Pass Port Cell-Jaunpur, which File Reference No. is LKO-Z941665-10.

After that first LIU inspection and then Police enquiry through Police Station-Sikrara have been done. When I enquired my Pass Port Status on dated 28.09.2010, I saw that status is OK and when I enquired inquired on dated 06.10.2010, I saw that “Please explain the circumstances under which you have given the false information/suppressed the material information and why action should not be taken against you under Pass Port Act-1967, Please send your old passport”.

I would like to inform you that neither any Pass Port was issued to me nor I have given any false information in Pass Port application, when I have no knowledge of previous Pass Port acknowledgement no., it is not possible to mentioned in application form.

All the information/material information given in Pass Port Application related to me are true of best of my knowledge. Neither I have given any false information nor I suppressed the material information.
