Dear Sir,
I have applied for passport on 26th June, 2008. But there was an
objection raised on my file (File ref. No. Z011096 / 08) on 2 Personal Particular forms. I have submitted the 2 Personal Particular forms duly filled in and affixing photographs along with attested copies of documents
on 14th November, 2008. But there was no change in the status of my application. Again I was informed to submit the 2 Personal Particular forms which I have submitted on 24th February, 2009. Since there is no change in my passport application form, I have re-submitted the 2 Personal Particular forms duly filled in and affixing photographs along with attested copies of documents again.


The authorities at Puri SP office says that they have already sent my police verification to Passport office, Chennai. As per my request they have sent my police verification again ( on 26/11/09) which bears speed post no: EO333744367IN.

Kindly look into the matter personally and let me know if I need to submit any other document.

Dear sir,

I have applied passport on 25.03.2011,with reference to your letter dated 28.09.2011, reference file no;MASP00145211,I have already furnish the detail for my initial expansion.
after that when I checked in Online and found passport will be shortly despatched.
It is suprise to see again you are asking for same.I have received the letter dt 28.11.11
for the same reason.File no: MASP00145211.
I have changed my initial as M.menaga instead of V.M.Menaga.I also enclosed my affidavit
along with my ration card and voter Id by register post on 13 oct 2011.
so i humbly request you to issue my passport without any further delay.
