Sir, I have applied for pan CARD which was issued to me accordingly and the details is as follows:-
Name :- RUPAK RAJAK
PAN :- AZTPR7415E
Date of Birth :- 01-03-1973

on getting my Pan Card I find that the date of birth has wrongly written as 01-03-1973 which ought to be 01-03-1978. Immediately I returned the Pan Card by Post with application to correct my date of birth and re-issue the same. But unfortunately till date I have not receipt back my Original Pan Card. Now i am facing great difficulties.
Therefore, I humbly request you to kindly take up my matter and return my card early. And for this act of kindness I shell remain grateful.
