DOB- 01/03/1979

But the My name on my PAN card is appeared MANISH KUMAR SHRIWASTVA
Plase change My name to:
MANISH KUMAR

Every where is I use only MANISH KUMAR.
Passport, Driving Licence, Bank Account, Across all education certificate.

Also please change the address as below:
