Respected Sir/Madam,
Subject: Requisition for the Cancellation of Health care policy. Policy No : HN25122009000562
Due to Some personal resaon, I would like to cancel the Health Care Insurance Policy HN25122009000562 from HDFC Ergo which has been renewal on 25, Dec 2011. Since the Policy has been added in my HDFC signature credit card, I request you not to debit insurance amount from my credit card account. Thank you for your timely consideration in this matter.
