Respected sir/madam

I have an account with state bank of mysore and my account number is 64014967169. I have applied for internet banking username and password around 2 and half months back. i have reminded bank twice that i have still not received my username and password but i am not getting proper responce and i suffer a lot due to this problem. My two complain number is sbmcompl00003514 and sbmcompl00003575.

So please kindly look into this matter.
