I lost my bsnl sim about a month ago no. 9457766488. I request you to please block it.

I have lost my mobile phone vodafone SIM on 15th February 2012 somewhere which No. is 7838448640. When I go to police they ask to present an affidavit proving that I am the authorised user of the lost SIM.
Unless I have a received complaint copy I cannot be issued the duplicate SIM. Please advise what shall I do ?
