    Dear sir,

    I have taken a mediclaim policy of The oriental Insurance Co Ltd at vadodara branch in 26/6/2006 and my policy No. is 171101/48/2009/1027 and insured 's code is 13529038. I was hospitialised due to suffering from sinusitis + delirium + meningiencephalitis and I was hospitliased from 30/4/2009 to 4/5/2009 at Faiz Nursing Home Vadodara. But in Mediclaim Medical Report the subordinate of doctor has written the wrong date in point No.5 When did the patient approach you for the first time in connection with present
    disease suffered ? The date written is on 4/5/2009 and Date of first consultation : The date written is 4/5/2005

    The above date was written by mistake and it was wrtten by the subordinate of doctor
    for which the doctor have given the certificate the patient has approached first time for disease on 28/4/2009 which was subimtted to MD India Healthcare Services Pvt Ltd at Baroda office.

    Even in M M R the second point my birth date was also written wrong it was
    written 2071. and in eleventh point Is disease suffered Acute or cronic ?
    They have written Acute and hence it is not chronic.

    For the above given reason the MD India Healthcare services is not passing
    my claim and I have done appeal for three times but they have rejected my
    claim after giving the certificate of doctor for mistake done by his subordinate.

    While taking policy on 26/6/2006 there is no any pre existing diseases mentioned in
    the form.

    Despite of done three appaeal at MD India Healthcare services at Vadodara office.
    They have rejected my claim of Rs. 17000/-

    It is my kind request to look in to this matter and reply positively.

    Dear Mr. Kazi,


    Kindly send a written complaint to the following address and wait for the response for 15 to 20 days and revert back soon with the reply from the Co. Just keep a copy of that letter, so that if the Co. is not going to help, you can forward your complaint to RBIs Insurance Ombudsman division to get any help on company level. The address is:

    Angry claim money

    Dear sir,

    I have taken a mediclaim policy of The National Insurance Co Ltd at Delhi branch my policy No. is 360303/48/48/8500002078 . My wife was hospitialised due to suffering from co breast/Rb.) sx(hrm).She was hospitliased at joy Nursing Home Rajouri Garden , New delhi. We claimed on 2nd june 2009 at Karam pura branch
    Even we apparoched to kolkata branch, but we did not got any response towards them.


    It is my kind request to look in to this matter and reply positively.
