My name is Yashpal Sahdev working in DIGHI PORT LTD.
Our company has provided us a mediclam policy by PARAMOUNT HEALTH SERVICES (TPA) PVT LTD (INSURER - HDFC ERGO GENERAL INSURANCE COMPANY LIMITED)
My Policy no. is 2999200076133100000 & PHS ID – HC MUM 2603389 Digh E valid upto 31/03/2012.
I fell ill on Dtd. 03/09/2011(Saturday) having severe chest pain. Due to emergency I have to undergo hospitalization in LIFELINE Hospital, Panvel. I have intimated PHS on Dtd. 03/09/2011 (Saturday) about hospitalization with intimation no. 5138040.
I have to be admitted in emergency & then after stayed for 24hrs in I.C.U. Igot Discharged on 05/09/2011(Monday) .
I have submitted all documents for processing of claims but now after almost 2 months insurance company is saying that we can’t clear your claim. They are saying that they have blacklisted the hospital which we were not aware earlier.
I request to do the needful.
