The purpose of this communication is to draw your kind attention to the fact that a claim for Rs.19499.00 was lodged at disposal in connection with the captioned claim details against the treatment for above noted diagnosis. It is observed that deduction for Rs.18328.00 has been made against my claim amount of Rs.19499.00. Therefore I requested NATIONAL INSURANCE to furnish the details/summary of the deductions with reasons thereof for the same. I am not really satisfied whether the deduction and settled amount has been done as per the terms of the policy.
You are requested to check and recalculate the claim bill so that deductions be minimized. Your early gesture in this regard is being desired as a good deal time has been already consumed at your end to settle the claim.
