Subject: Claim for Medical Expense.

Herewith Iam attaching the medical expense claim file foryour reference.

I had taken a reliance health policy on the internet through my credit card. Please refer the policy document for further details.

My policy no.1306592825000009 silver plan. I went to the medi Assist’s office in Ahmedabad to submit my claim file and they were unable to find my policy information on their system database. They said “Sir this policy is issued from Delhi hence you have to send all the original document via courier to Mr.Faisal Khan for the further process in Delhi”

Hence I am sending all the supporting document for the process.Kindly look into the matter to clear the claim.

Your early response will be highly appreciated.
