Subject: Harassment in Medical and Death insurance claim from Oriental Insurance Company(OIC), New Delhi under Nagrik Suraksha Policy and GPA policy.

Respected Sir,

1. With due respect, I beg to lay down the following few lines for your kind consideration and favourable action please.

2. With deep regret this is to inform you that my mother passed away on Oct. 2009 due to a massive Brain stroke. We have filed all documents for claim, 2009 to Oriental Insurance Company, New Delhi office against the policies of my mother:

1. Nagrik Suraksha Policy No.:271700/48/2009/1713 Cover Note No. :270000381493 Certificate No :OIC/NSP-9342 Date of Issue : 2/1/2009. Amount Rs. 2 lac 50 thousand.

2. G.P.A. Policy No.:271700/48/2009/1714 Cover Note No :270000381494 Certificate No :OIC/GPA-9342 Date of Issue : 2/1/2009 amount: Rs 3 lac.

3. After no response from them, we enquired in March 2010 from the Customer Service Department, OIC about the claim. They assign query ref no. CSD/ NDR-II / 41 / 2010 dated 3/3/2010. And after one month on dated April 19, 2010 they respond stating claim lodged for death of my mother stands repudiated due to non submission of the following documents:-

1. MLC.

2. FIR

3. Post mortem report

4. On dated April 20, I have replied stating the non-availability of the above documents as:

1. MLC

Explanation: As we have already explained the cause of her brain stroke. On Oct 19, 2009, it was a house hold head injury (slipped on a wet floor). The first aid report from a local hospital along with a letter of Pardhan, Gram Panchayat Bharmar (as first information) was attached in context with this information.

2. FIR

Explanation: It was a house hold injury, so there is no question of Police report and FIR.

3. Post Mortem Report

Explanation: On medical grounds, the case was referred to different hospitals consecutively. Finally my mother’s death was declared from Apollo Hospital , Ludhiana. The final discharge report along with the cause of death was also enclosed with the claim documents.

5. There were no medico-legel issues on the cause of death, hence no Post Mortem was done.



Even though nothing can repair our loss but since I was dependent on her and I was the nominee of the policy.

Inspite of their all queries explained they have closed my file. We have send all the documents, medical bills, death certificate, first information from our Gram Panchayat Bharmar and policies details.

Now, they are now not cooperating me neither they are responding my emails and as mentioned above these above documents are meaningless in the present case and now they are simply harassing me. I have borrowed about Rs 3,00,000/- (three lac ) money from my relative for caring my mother. I am now in big trouble. It is now more than 6 months.

You are my last hope. May I request you to please look at the matter.

We look forward to a quick resolution to our claim.
