    Dear Sir,

    I met an accident on 23.05.2009.at 9.30am.at our residential building terrace and
    Head and back injury immediately admitted in nearest hospital. Admitted in the
    Hospital two Occasions. (23.05.2009-30.05.2009. and 04.06.2009-08.06.2009.)

    On admission in the hospital I informed customer care support Icici Lombard on toll
    free no.1800 209888.and they given me claim no.406091405/04.06.09.and also told me
    claim will be settled within receipts of 45 working days. On phone and the also sent
    claim form within a week. After improving my health I duly filled the said form and
    attached all hospital document, discharge certificate issued by doctor and all
    prescription and medical bills sent for reimbursement through First flight courier
    Vide docket no M48251944/03.07.2009.and first Flight courier deliver said document
    on 06.07.2009.at ICICI Lombard General Insurance company limited.Icici Bank towers,
    bandra Kurla Complex.Bandra (East) Mumbai.400 051.

    On 21.07.2009.I received call from Mumbai.tel No.022 39839443.and enquired about how
    the accident is happened and I also cleared about the same. Thereafter concerned
    person told me your claim document will be sending for investigation.

    On 30.07.2009.I called customer care support on toll free number at around 5.11pm
    and spoken with Mr. Anthony at Hyderabad and Mr. Anthony given me request
    No.3007091238.and also informed me phone/sms/and sent mail mentioning your claim
    will be settled 21 working days.(mail proof if required I can submit at any time.)

    On 31.08.2009.considering 21 day completions I called customer care support on toll
    free number at 9.48am.and spoken with madam sunita and sunita telling me your claim
    will settle next 7 working days.

    Yesterday considering 7 days completed I called customer care support and spoken
    with with madam Laxmi and requesting me wait for 7 working days.

    Now already two months completed no sign of any claim settlement when called from
    Hospital I informed that claim will settle within receipt of 45 working days now
    every time I am getting extension. Can you help me for the same?

    Default ICICI Lombard Medical Insurance

    ICICI Lombard Medical Insurance have not been paying my medical
    reimbursement bills of Rs.3 Lak approx.since last one year.However,I
    have send many reminders to ICICILOMBARD.Sir,Madam I met an
    accident.(copy of FIR attached) as on dated 26.5.2008 at 1.20 pm on
    NH-8.I was quite well before accident now I am suffereing from Chest
    ache,ribs ache.Shoulder ache all time.I have no money to pay doctor
    bills.The doctor has advised me to take surgery of Chest.I have Loan of
    Rs.5 Lakh also.So,I have requested to you pl.do something for me.

    Hope,you will do consider my request.
