Please refer to the attached mail and trailing mail, where I have requested to change the Policy payment term as agreed while issuing the Policy to me, which was wrongly issued to me for regular premium.

I agreed that the signature on application is mine, but the person send by Mr.Vijay (Broker) for filling the application, he has taken only signature on blank paper by saying that rest of the details I will fill and requested that i have attend some other client also ( On Cheque copy signature is attested). And on the faith of SBI and agent I signed the application on blank paper. When the policy was send to me at that time I was on official visit to project site and came back after 21 days. After seeing the policy paper I requested immediately to broker as well to SBI.(Policy paper attached for your reference).

There is no proper feedback given to me by SBI Life and by agent (Mr.Vijay) from last 1 month. I tried with Mr.Raina (SBI Life) and broker (Mr.Vijay) to get the issue resolve.
But today I received a letter stating that the policy cannot be cancelled.(Letter attached) and broker saying that please not to call me.
Here I am not asking for cancellation of Policy but asking to change the premium term to be read as single premium payment up to 5 years only. This is because I cannot afford Rs.50,000/- as premium every year.

I hereby request you, please change the payment term to single premium. The ID No, Email ID and Contact no of broker Mr.Vijay is mention in attached mail. Some of the evidence is also attached for your reference which was given by broker while issuing the policy.
