Dear Sir / Madam,

I regret to inform you that after submitting all necessary Form / Paper on 5th Feb. 2011 for above pension scheme in option ‘F’ for monthly ECS in ICICI Bank Borivali (west) Mumbai for Rs. 7526.00.

I regret as on today NO POSITIVE reply from any department of L.I.C. / Brach /Office to solve the problem

I also regret that your (888) Branch has done silly mistake to Registered the yearly Payment instead of MONTHLY Payment fill up in the form, I hope you will rectify the internal paper work expeditely and do the needful immediately.
