Dear Sir,

One person Mr. Rajesh Nair from AVIVA Life Insurance Company Limited approached me
for insurance policy for children. I was impressed with policy details as shown in
the presentation. I took two policies for my children. They have done some mistake
while issuing policies. Then they had cancelled the policies and issued new one
against the same payments. I had given them ESC facility to clear my monthly
installments. After cancellation they have taken money through ECS for cancelled
policy no AYS2386531.

They had issued new policies without taken my signature on new applications.
Applications for new policies I had not signed. In future I will face another
problem while policies claim for disbursements.

While cancellation, how much money I have lost they are not showing in their
statements. I am asking for details, but they are giving only assurance.

I had asked for refund of this money, but they are not giving. They are only
promising, but doing nothing. Hence you are request to help me in this regard.

Please do the needful in this regard.

Dear Rajesh

I have not yet received my refund cheque till date against cancelled policy no
AYS2386531.this policy was cancelled in free look period, but after cancellation you
have taken very next installment through ECS. I have received SMS from your side
stating that cheque is dispatched through Blue-dart AWB no is 4357751625, but I have
not received this cheque till date.


Kindly arrange to send my cheque of Rs 2500.00 ASAP.
