This is with to my Kotak Smart Advantage- Life insurance policy. The quires which i had put across for with Kotak Life insurance, instead of constant follow up my concerns were not taken into consideration.
Following are my quires:
• The name which is mentioned in policy is not appropriate: Name is the personal identification of a person but my policy doesn’t display my Name properly- My name SUDHIR H B. But, in the policy my name has been mentioned as SUDHIR BH.
• My Father name is not Correct: My FATHER name BALAKRISHNA H S. But, in the policy my father name has been mentioned as BALAKRISHNA SH.
• My photograph is not main: There are list of complaints to put it across how can executive be so very careless and put some one else photograph in my policy, because none of my original documents are correct.
• My Signature has been forged: The Signature which ever mentioned in the policy is not mine some body else have been forged my signature.
When I have contacted the company in Bangalore with all these issues on 18-Oct-08 with summiting of my original policy document; I came to know that I need to give a new application form with duly signed.
An executive comes and collected the photo, Driving License, Pan card copy and got signature on the two page unfilled application form on 23- Oct-08; and also conveyed that I will be getting a call to fill up the application form. But, so far I haven’t received even single call from any executive from Kotak Mahendra.
Till date, I was in the assumption that my quires has been taken care and the corrected policy will issued with in 15 days of time on the date of collection of photo and related documents.
But, when I gave a call on 17-Nov-08 to Kotak Mahendra costumer care i have been informed that there is no application form or request has been reached so far. They have given the wrong reference or complaint no (No- 01266576). But, Deputy Manger helps me out the lodge an complaint on 19- Nov-08 (No- 01269828).
After all these effort she pledges me that sales executive will get in touch me to rectify the problem. But there is a no call from there end till date.
On 20-Nov-08 I had posted an web complaint (clientservicedesk@kotak.com) on all these issues I got an confirmation mail on Dec-01 from Asst Client Service Desk Manager by stating that my all these issue has been rectified and updated policy will be dispatched with in two working days.
Till date i had patience of following up with the executives since from two months but there is no updates from there end.
I request u to please help us in finding a solution for this problem.

I am amrendra kumar amar policy no-02171089 and policy name is kotak surakshit jeevan. My complain is the name of insured is wrongly printed in policy document as rubby kumari where as it should be rubee kumari.please correct the name of my insured wife as rubee kumari in main data base.i also submitted the certificate of my insured wife.
