Dear Sir/Madam,
Sub: Issue of the Sum Amount to the Life Assured for Contract No:
11836584, Client ID : 03923572.
This is in reference to Reliance Automatic Investment plan - Reg,
Policy No(11836584) drawn in favour of MRS. Kacham Nagaratnamma and sum assured worth Rs 100,000/- and as a premium of Rs.10,000/- for year.
The above said policy was proposed on 31-march-2008, While the time of
proposal client (MRS. Nagaratnamma) given the correct correspondense address and mobile number for further contacts and also it was same on Application form as well, but while issuing the policy bond and other receipts there was a mistake on correspondense address and mobile number, So that cliant was not intimated for the second premium.
Because of this communication gap client was not made the second premium payment 10,000/ on or before the due date which is 31-March-2009, then in the mean time client was died with an road accident on 03-June-2009, and Policy nominee also severely injured and he went for a big surgery.
There after relatives of client was given the death intimation to the nearest branch manager and submitted the related documents Form A, and Form B including Post Martem Report, FIR, Nomminee's Bank Account number and ID proof.
The Reliance Branch manager forwarded those document to the head office and he received the cheque for an amount with current fund value of Rs .5,419.74/- .
But policy holder was died with an accident and nominee suppose to get the sum assured of Rs. 1,00,000/-, because of wrong address and mobile number entry in the policy documents he/she was not intimated for second premium.
For this entire process client ulimately suffering with our miss communication..
Note: The Client and the Nominee both were uneducated people and they were not aware of the wrong address printing on the policy bonds.
