Kindly refer to my application regarding cancellation of my policy number 856383658.Despite of my several request on your tole free number -18002004006 printed on the cover of above captioned policy number bag .I purchased the above policy through AXIS BANK ,GORAKHPUR.I received the policy bag on 27th October 2011 .The terms and conditions incorporated in policy bag is not acceptable to me.Therefore i decided to cancel my policy and contacted AXIS BANK ,GORAKHPUR .I gave a hand written letter and filled a prescribed proforma provided to me by the bank for cancellation .My application along with policy bag was received by Mr Azam Khan,GKP0137.I applied for the cancellation of the policy within a week.As per IRDA guidelines and as per your policy conditions too i have right to cool off or cancel my policy by applying for cooling off and surrendering policy bag along with duly filled proforma for cancellation .It's most surprising that I have been misguided by your representative through the said tole free mobile number 18002004006 and the current status of my policy is still not communicated to me .Let me know whether my policy has been cancelled or not .
You are therefore requested to consider the matter seriously and refund my premium at the earliest otherwise I have to complain against MINISTRY OF FINANCE, INSURANCE REGULATORY AND DEVELOPMENT AUTHORITY AND OTHER COMLPAINT REDRESSAL CELL.
