    Sub –Complaint for non-reciept of Matured Claim under Policy No.304835(Issued by Metropolitan Insurance Co Ltd)on own life of Sri Anirudha Panda.

    Sir,
    I have moved the Insurance Ombudsman,Orissa,Bhubaneswar complaining non-receipt of my matured claim under the policy No304835 vide my letter dated24.10.2009(copy enclosed) addressing to the Secretary,Office of the Ombudsman,62,Forest Park,Bhubaneswar.

    The Deputy Secretary,Insurance Ombudsman,Orissa,Bhubaneswar has informed that the matter comes under the jurisdiction of Hon’ble Ombudsman,Kolkata since the premium was being remitted to Kolkata Branch.


    1.In view of the above situation,I herewith file my complaint with a request to settle my
    matured claim relating to Policy No.304835 at an early date.

    2.Further I enclose herewith the copy of letter No.CD/Claims/Misc/AO dated 27.01.2010 of the Manager(Claims),Orissa,Bhubaneswar in which he has informed that my representation has been transmitted to the Manager(Claim)Kolkata Metropolitan Office-1,Jeevan Prakash Building,Kolkata-72 for redress of my grevence.

    3.Accordingly I preferred my claims to Manager(Claims),Kolkata vide my letter dated 28.01.2010(copy enclosed) and I am yet to hear from him.

    4.The delay in preferring the claim to Manager(Claim) and filing of complaint before you is due to ignorance and for want of proper guidance from the officers of Orissa zone despite several representations to various quarters.

    At last I pray you to kindly consider my pathetic old age condition(83 years) and pass favourable order to release my claim which will be a great help to me ay my last stage of my life.

    This was the letter in 21.04.2010.There is no response.Pay my Insurance money within may31st.Or else I will go to media and file a case against you.

    Default Lic fraud

    Its been two years since my father Mr.Devendra Ratilal Pitroda passed away and I filed for claim through our insurance agent Mr. Viral Fadia and have not recieved any claim disbursement or any notices of the same. I have only recieved a Cheque dt.30-3-2009 in favor of my father Devendra Ratilal Pitroda who passed away on 19th april 2008. The disturbing fact is that the cheque of Rs.8824/- was hand delivered by the insurance agent yesterday on 26th april 2009. And along with that i have recieved a letter from LIC stating that this policy has been surrendered.

    LIC acknowledge the receipt of my GRIEVANCES at their website.
    The GRIEVANCES number is 178625

    On 17 December 2007 when we came to know about my fathers ailment we contacted Mr.Viral Fadia (LIC Insurance agent) for the same and asked him if there was any medical policy to cover my fathers ailment. He replied there is not any policy which will support the cause. He lied at at the very beginning because my father had a policy no.901069526 Which is a Asha Deep LIC poilcy and covers my fathers ailments which we found out later this year. While ongoing treatment of my father at bombay hospital which he was diagnosed for cancer and operated on a brain tumor, we hoped that Mr. Viral Fadia would have helped us on filing the claim of the above policy which would have helped us immensely during the traumatic period we were going through.

    Later on while being discharged from Bombay Hospital after having 4 chemotherapy treatments my father was advised for complete bed rest. As he was not able to walk and was severely disabled due to the chemotherapy sessions our business was going in losses which only my father used to handle. We went through emotional turmoil at that stage. Our family has gone through a lot of pain seeing my father suffer and at that time Mr.Viral Fadia would have helped us. But he chose to not.

    On April 19th 2008 My Father Mr. Devendra Pitroda expired and the same was reported to Mr Viral Fadia along all the relatives of ours. After completing my fathers ritual ceremony we asked Mr. Viral Fadia to process the claim for my father who was life insured by LIC. I personally ran from pillar to post getting all the necessary documents from hospital because Mr. Viral Fadia had said these documents were required to file for the claim. At that point of time I asked Mr. Viral Fadia about my fathers policies and asked him about what is the sum assured we will get. He deliberately lied at that time also by saying there are two polices where we will get roughly around 2.5 lakhs. He also lied that there are complications in my fathers case and there woulld be investigations and this would take time of around 2-3 months. We having no where else to go and trusting Mr .Viral Fadia for being my fathers LIC agent for more than 15 years waited patiently and kept on following him up at every middle and end of the month.

    My father was life insured by total of 7 no.s of policies :- 900680636, 900686596,
    901069526, 901073670, 904307681, 904990748, 905214961.

    No action is been taken against lic agent viral fadia. Agent code-40919. He forged my deceased father's signature in order to surrender the policy which was to be claimed. Policy no-900680636. My mother is the nominee in this policy. But still after having complained to the branch, sr.div.manager, zonal office, they have given me a form of open title nomination.
    My father has expired on 19th april 2008. Its more than 2 years, i am still following up to get the death claim of my late father. Lic has not been helpful instead they are burdening me.
    P.s: The agent viral fadia is still employed by lic. No action has been taken against him for forging and surrendering my late fathers policy.

    Default Not reflecting my premium paid

    Dear Sir,
    My policy nO. 153212803 has been starting from jan 2009 and premium paid from the same date. but in my account premium shown for the year 2009-10 & 2010. Where has gone Rs. 5000/- which is given in the month of Jan.'09.
    Let me know urgently othewise i will lost believe on LIC.

    Arrow claim not setteled

    I lakwinder kaur w/o late inderjeet singh badesha. My husband late inderjeet Singh badesha has taken a policy no.300826875 on the name of my daughter jasleen kaur & policy no.300826876 on the name my son armandeep singh on which my husband late inderjeet Singh is prosper in 2006. The lock period of this policy is three yrs. Unfornately my husband is died on 16-1-10. I requested the lic office unit-5 ghant ghar chownk Ludhiana to change the prosper but the refused to do so. Now I want to surrender these policy’s as I am the legal heir of my late husband .I visited the office the tell me that the policy is surrender by the title open I submit all the required documents on 28-4-11. But whenever I visit the office the tell me that my case has been sent to divisional office the harassed me for the no fault of mine.
