Dear Sir/Madam,

I the above named person, need help in addressing a problem
with Columbia House Dvd Club of PO Box 91601, Indianapolis, IN 46291-0601,
their E-Mail Address is @columbiahouse.com , and Automated Phone
number is (800)-262-2001.

Around February 10th, 2009, I was taking part in
an Online survey which lead me to Columbia House Dvd Club website for which
participation
was required and an offer to purchase movies was made. I
purchased seven movies via the Website and provided my Mastercard Debit Card
information and agreed for $27.35 to be deducted from my
attached Account, this was done with no problems.

However around March 23rd, 2009, I received another movie from
Columbia House Dvd Club which I did not order and a statement showing $25.94
was deducted from the Mastercard Debit Card. I contacted
customer service via their above Phone number and spoke to a female
employee with a
heavy Middle Eastern accent, who refused to give her name, and
inform her of this additional movie and the deduction. She informed me that
when
I purchase the first set of movies, I automaticlly join the Club
membership. I informed her that no one inform me that I was joining a Club
membership
and as a result which to cancel any membership.
On April 13th, and April 27th, 2009, I again received one movie
on each occasion and $25.94 each was again deducted from the same Mastercard
Debit
Card. I returned all three Movies, which I did not order, sealed
in their original packages, to another mailing address PO Box 19004,
Greenville, SC. 29390.
This address was supplied by the female with the Middle Eastern
accent.

May 15th, 2009, I contacted Columbia House Dvd Club and they
confirmed that they received the three returned Movies, however I was
informed that I
need to pay an additional balance of $74.82 to end my
Membership. Since then I made many attempts to contact a Supervisor or a
Manager to be enable
me to address this matter but informed that they have my contact
information and would contact me at a later time. Three notices since came
to my
address informing me that my account # 50569431583, would be
reported to a Consumer Reporting Agency.

Wednesday 7th, October 2009, I received a letter from a Debt
Collection Agency named Allied Interstate of # 540 Dick Road, Suite # 100,
Depew,
NY 14043 and Phone number (800)-758-9849, informing me that they
will obtain verification of the debt and obtain a copy of a judgement which
may be against me.

I hereby ask for an intervention please, due to this Columbia
House Dvd Club, did not inform me of a Membership to join, before I ordered
the First
set of Movies. The other movies that was sent to me was not
ordered by me and was returned, but a total of $77.82 was deducted from my
account
and not returned so to stop any further unauthorized deductions,
my Debit Card was changed.

Thanking you in advance for your assistance in this matter and
pray that a speedy resolution can be reached with all Parties involved.
