Dear Madam/Sir,

As desired by you, I am herewith sending you signed copy of Credit Card
Dispute form.
Here I want to bring one thing in your notice that the policy
subscribed (Policy No. MRP11000005587) by me on 25 Nov -08. I thing merchent
(TATA-AIG) has claimed the amount to my bank (Axis on 28 Nov-08 of Rs.
7038/- with transaction Ref. No.27384824. After the cancellation of policy
in the month of Dec-08, the refunded the amount of Rs. 6939/- with
Transanction Ref. No.28047756 after deduction of Rs. 99/- for service charge
on 13 Dec-08.
After two month (Means in Feb-09) how once again the claimed the amount of
Rs. 7038 for the same policy with Transanction Ref. No. 31120351 on back
dated. I am unable to understand how they claimed the amount once again
after cancellation of policy with only one permission.
So, I request you to kindly see the matter & treat it most urgent as the
payment due date is 04/04/09 only. I am unable to pay the amount as already
one EIM is going on with your bank.

Policy No. MRP11000005587. (Axis Bank Card No. 4718630003304218)

Dear Madam/Sir,

As per the above reference No. & policy No. TATA AIG had to refund the
amount of Rs. 7038/- within 7 working days from the date of complain. But
till date I haven’t received any amount from TATA-AIG.

Due to delay in refund of amount, my Axis Bank has charge me Rs. 1242.89/-
with 10.30% Service Tax. I my earlier communication with TATA-AIG I had
clearly mentioned that, if they will not refund the amount before 7th March
2009, bank may charge for delay.

Here I want to once again remind one thing that this fault is not from my
side. This is either from TATA-AIG or Axis Bank. As I subscribe one policy,
how the TATA-AIG debited the amount two times for one policy on different
dates (i.e., on 25th Nov-08, & 28Th Nov.-08), and claimed the amount after
two months of cancellation of policy. At the same time, how Axis Bank
released the amount two times with one permission only. This thing shows
that online transaction is not safe or especially IVR system is not safe
because any merchant can claim the amount many times in an internal of
months or year with one the basis of one permission.

So, I request to you TATA-AIG & Axis Bank to sort-out the matter among us,
either Bank will have to cancel the penalty charged to me or TATA-AIG will
have to pay the penalty as this delay is from there side, because I was
unknown about this situation as TATA-AIG has claimed the amount second time
after two month of the cancellation of policy. Or please guide me where I
should raise the complain for this type of dealing between Axis Bank &
TATA-AIG.

I hope you both will treat the matter as most urgent & important & try to
finalize as early as possible.
