Default ECS mandate form not sent by AXIS bank

I got a loan from AXIS bank, the first EMI got deducted from my salary
account by cheque, but the second and third got returned as AXIS had
not sent the ECS mandate form to my bank. AXIS claims they've sent it,
and that my bank has refused to accept it without giving any suitable
reason. This has spoilt my credit history, I had no cheque returns ever
since I had this salary account opened and now I have two in one month.
I sent emails to AXIS's mumbai n gurgaon branch, spoke to the credit
manager, addressed the people giving collection calls. When I got no
answer, I finally complained to the RBI, post which the credit manager,
and branch manager have both called to resolve the concern. The want me
to speak to my bank, which I dont think is my job, as they didtake the
file processing charges. I intend to send them a legal notice....any
    suggestions

ask axis bank to send a new ECS form. THe process is complete when you receive
SMS or email for ECS mandate from your bank. may be it was lost.
