amount debited but cash not recived

I MEENA KUMARI tried to withdraw money from a PNB ATM branched at MANDI,himachal pradesh a RS 15000 amount was debited but no cash recievd. My a/c no. is 1408000100104953 and ATM card no. is 512652*******673 so kindely credit my account or get back to me my ph no. is 9418200633.

I used STATE BANK OF INDIA debit card no.- 6220181030800059442 for cash withdrawal in YES BANK ATM at MORADABAD (ATM ID NMOWN135).

i received 10000 for the first withdrawal, but in second transaction i entered 7000 again, my account balance was deducted by 7000 but i dont received the cash from the ATM.

I humbly request you to do the needful and refund the amount which is wrongly deducted in my account.

If you are not the right person, please let me know the concerened person to contact for this problem. please help...
