wrong debit due to atm denial for Rs 4000 on account no.0687000109963607

Sub :- Non Receipt of Rs.10,000/- from ATM PNB Khanpur -62
Dear Sir,

I am a satisfied customer of PNB bank . I am having a A/C in PNB .My A/C numberis 4881000100030290 . I am using yours bank ATM Debit Card . On 10th July 2011, I went to withdraw cash from an ATM PNB (Raju Park Khanpur) & using my PNB DEBIT CARD at around 8.10 AM, the transaction was turned down and no cash was received. Again I went to the next PNB ATM (Duggal Colony) and could withdraw money( Rs 10,000/=) at 8:20 am on the same date .(Both the ATM's are next to each other located at khanpur Devli Road New Delhi- 110062.)
on 11-7-2011 I went to get my Statement of account, To my surprise the account in my passbook shows two ATM withdrawals of Rs10,000/= each on the same date..
Alternatively, we made a Complaint to Customer Relationship Centre on toll free number 1800 180 2222, our complaint no. is 55758922 dated 13-7-2011. When I follow up with call centre person they told me your complaint will be rejected and talk to your branch manager. I wrote a letter to branch manager on 21-7-2011& explain the matter.
Then Manager recieved a fax copy of statement from ATM Department, Statement shows that only one transaction between dated (10/7/2011 to 12/7/2011) but still amount deducted twice

You are requested to look into the matter and recover my money (Rs.10000/).
Do the needful at the earliest, we urgent need of funds.

i have a saving account in bank of Baroda ( kareli branch Allahabad) a/c no. 19210100003661.... on 10th of November i used ICICI kareli branch ATM to withdraw 2000 rs ... but at the time of withdrawal there was some technical issue and money was debited from my ac but it didn't got dispensed , i approached my bank and i was told it was normal and money will return i 48 hrs ... i waited and watched for 7 seven days then again i approached bank of Baroda they told me to write a written complain and told it will be credited in 10-12 days , i approached the bank after the due date they told that i will not get the money as ICICI bank has said that their ATM has dispensed cash ... pl z help me in this case i dont know what to do ...plz direct me ... will be highly obliged ...
